Sun Safe Summer- How to protect your skin!

Many new patients come to me asking for fillers Botox etc. We all know that prevention is better than cure. That’s why one of the first questions I ask is How do you protect your skin??

Well, most people use skincare that’s high street and its not really that effective in protecting your skin. It’s never too late! The first protection I would recommend is a physical sunscreen that factor  40 plus and  that’s free from chemicals and that can protect your skin effectively all year round.l

Surprisingly, many people are unaware of the dangers of the sun and its detrimental effect.   So here’s Some advice to  enjoy outdoors safely.

We all look and feel better with a hint of colour but what is the SUN really doing to our SKIN?

The 6 things that influence skin ageing, laxity, pigmentation and skin cancer.

Most factors we can control, however some we can’t.

UV radiation is made up of UVA and UVB rays which are able to penetrate the skin and cause permanent damage to the cells below: UVA penetrates deeply into the skin (the dermis) causing genetic damage to cells, photo-ageing (wrinkling & blotchiness) and immune-suppression.

  • UVA rays penetrate deep into the dermis, the skin’s thickest layer. Unprotected exposure can lead to premature skin aging and wrinkling, and suppression of the immune system. These rays can penetrate through glass so we cannot avoid it. UVB rays will usually burn the superficial layers of your skin. It plays a key role in the development of skin cancer.
  • Decades of burning and cumulative exposure may cause Skin cancer.
  • Damage in childhood causes elastosis, premature wrinkling and age spots.

As we age the skins ability to rejuvenate lessens. Childhood burning on at least 6 occasions increases the risk of skin cancer. Throughout childhood our skin cells can renew but as we age the body’s ability to repair diminishes resulting in mutations and skin cancer. This is why generally age spots, pigmentation, deep wrinkling and skin cancers appear over 40 years.







86% of Malignant melanomas are linked to UV damage and sunbeds 10 times stronger than the midday sun. Using sunbeds for the first time before the age of 35 increases the risk of developing melanoma skin cancer by 59%. Regular sunbed use under the age of 30 increases the risk of skin cancer by an alarming 75%. The most common misconception is the vitamin D debate. For most people just going about their normal business in the summer months is sufficient to produce the required vitamin D levels. We certainly don’t need to sunbathe to make enough vitamin D. We can also get vitamin D from certain foods including milk, fish, egg yolks, and fortified cereals. Another common misconception: that sunbed use prior to going abroad builds a natural defence to the sun’s rays, the fact is that UV exposure from sunbeds is exactly the same as the damaging UV rays produced by the sun.

The basic rules PROTECT

  1. Wear long sleeves a hat and stay out of the sun especially between 12-3
  2. Wear sunscreen daily even in the winter months
  3. Reapply a sunscreen regularly depending on exposure
  4. Use a reflective based natural sunscreen as chemical sunscreens can be absorbed into the bloodstream rapidly
  5. Do not sunbath, as when your skin goes brown the DNA and dermal cells are damaged.

Sunscreens nowadays can be:

  • Environmentally friendly -Reflective sunscreen (physical)
  • Tinted – so you don’t have to use a foundation
  • Natural ingredients
  • Safe to use – non-toxic and free from oxybenzone
  • Easy to reapply- user friendly size

How does Botox™️ Work?

Did you know that Dr Carruthers an eye specialist discovered something spectacular

Botox™️ Smooths out Wrinkles

Before 1987 Botox was being used in traditional medicine. Medical issues such as muscle contractures, squints, eye twitching.

Dr Jean Caruthers used it to treat eye twitching. It was her patient who noticed a smoothness around her eye area. Light bulb moment! Dr Jean Carruthers teamed up with her husband Alistair who is a dermatologist. They knew they were on to something big. From then they trialled and wrote papers on Botox. which resulted in treatments to smooth out lines. Using it on the frown line & progressing to around eyes and forehead area. Later, other areas of the face became acceptable for use . areas suchas jawline, neck bands, dimpled chin, droopy mouth & bunny lines.

How does Botox™️ smooth out wrinkles?

So there are two types of wrinkles static and dynamic. Static which appear at rest .(without making and expression like frowning). Dynamic -which will only appear when e.g. frowning or smiling.

Static lines form over time, by using the muscles to make the skin wrinkle. Aging causes loss of strength and structure in skin. This causes the skin to weaken and the lines become ingrained in our skin. The skin doesn’t smooth out when we aren’t making that smile or frown. So these wrinkles become visible even at rest. If you treat these with Botox™️ the lines will not disappear but will soften over time.

Dynamic lines- these only appear when we make the expression. When the muscles are at rest they disappear. So if you’ve Botox™️ and you smile you won’t get those smile lines at the side of your eyes.

Botox™️ doesn’t work straight away it take roughly around 2-3 weeks to be effective.

You may feel your muscles feeling weaker over this time it lasts usually about 3-4 months.

A very common question I get asked ” Will it cause me to look older”? Research has proving ed, that once the effects of Botox wear off, your face will NOT look older. Actually, the opposite happens. This is due to the muscle not contracting and wrinkling the skin for a period of time.

Botox injections help you get rid of some of the unwanted wrinkles. If you have wrinkles around the eyes, forehead, chin, jawline, droopy mouth, gummy smile. it also helps with teeth grinding by softening the muscle that allows us to grind.

How to remove common moles & skin tags non-surgically

Moles no matter shape or size should not be removed without seeing your GP or medical specialist first.

Moles can appear anywhere on the skin. Most adults have 10 to 40 common moles. They form when pigment cells grow in a cluster instead of being spread out throughout the skin.

Common moles are usually small, round or oval-shaped skin growths with a well-defined border. They may be colourless, pink, tan, brown or black.

Non typical moles look different. If they’ve appeared suddenly, are irregular in shape are different colours, are itchy or bleeding they may be a cancerous lesion.

They’re generally bigger and tend to have irregular borders and colouring. Most atypical moles are benign, though they are more likely than common moles to become cancerous.

Helen our nurse prescriber will make sure you have been to your GP or dermatologist to examine your mole to ensure its cosmetic and no anything suspiciuos. If its a common mole then we can help at our Cheltenham Clinic .

Self-checking moles

While they generally do not pose a health threat, moles should be self-checked regularly. Schedule a skin examination if you find a new mole or a change in an existing one. A mole that starts to change or grow in size could be a sign of skin cancer.


Whether you choose to have a mole removed for health or cosmetic reasons, it’s important to know what you’re dealing with first. It may seem convenient to use over-the-counter products or home remedies to remove small moles. But it is wise to check with your doctor to make sure the mole is not potentially cancerous. Once your mole has been examined by your medical specialist and they have said it is non-cancerous then I Helen Cleland can remove your mole in a non-surgical way. We use a device which will leave you as if the mole hasn’t been there at all. The procedure is carried out in our Cheltenham Clinic.


The NHS mole removal service

As the NHS is beyond capacity they will not remove non-cancerous skin lesions so best to come to a medical practitioner who has qualifications to do such a procedure. With my background in burns and plastic surgery I am not scared to turn you away and refer you to the relevant specialist if I think the skin lesion is at all suspicious.

The way a Common Mole is Removed

With BT plasma, the energy uses plasma, a special form of a gas that has an electrical energy, to work throughout the skin from the epidermis deep into the dermis. The Plasma causes a process called sublimation which turns a solid into a gas so results are instant with moles, skin tags and warts being removed in minutes.


  • The procedure – a local anaesthetic cream or injection to numb the area
  • The plasma BT shaves off the lesion leaving a flattened area
  • As a result a scab forms and under this the wound heals
  • Scab falls off after about 1-2 weeks depending where it is on the body
  • Skin under will be pink and should be protected with a 40Plus SPF to prevent pigmentation
  • After about 4-6 weeks the your skin should go back to its own skin colour
  • Some cases the skin may appreat slightly darker or lighter than normal skin colour


The results are great. there is no noticeable  scar or unevenness or suture line. Results are immediate.



  • Keep dry for 24 hours
  • Do not pick the scab
  • Ensure the area is kept dry after washing
  • Once the scab falls off massage the area twice daily with bio oil
  • Use a high factor SPF on the area 40 plus
  • Do not use steam rooms, swimming or saunas until scab falls off
  • Do not pick the scab or this may cause uneven scarring
  • Do not undertake a strenuous exercise for the first 5-7 days




Not suitable for

  • People who suffer from keloid scarring
  • Pregnant or nursing mothers
  • Cancerous lesions
  • Certain medications
  • Blood disorders

What is PRP?

What is PRP

I have been offering this treatment in Cheltenham since 2017 and its a natural way to refresh and rejuvenate your skin with your own plasma.

PRP (platelet-rich plasma) therapy has been used widely in medicine mainly in orthopaedic and sports injury treating muscle and tendon injuries to help with healing and repair.

In recent years it has been used in the aesthetics industry to help with hair loss, scarring pores and skin quality.

Celebrities such as Kim Kardashian posted on social media a couple of years back claiming she had the treatment which boosted treatments worldwide.

Blood contains lots of nutritious and nourishing elements which may be used to rejuvenate the skin.

Blood contains proteins named growth factors which trigger wound healing and tissue growth. These growth factors may help condition the skin.

  • Softens fine lines and wrinkles
  • Reduce acne scars
  • Increase hydration in the skin
  • Stimulates hair growth


A blood sample is taken from the arm and then its spun in a centrifuge. This separates the blood into its component parts. The plasma separates form the red blood leaving an amount of the blood rich with platelets. The idea is that injecting PRP into skin tissues will stimulate your body to grow new, healthy cells and promote healing. The tissue growth factors are more concentrated in the prepared growth injections.

This is can then be injected directly into your skin, combined with filler, and injected, or applied to the surface of your skin following a microneedling or laser treatment.

So the good platelets increase the volume of healthy skin cells and help to plump up the skin and stimulates the production of hyaluronic acid to hydrate and plump too. The procedure typically treats the face, nose, décolletage & hands


The PRP facial effectively treats fine lines and wrinkles, acne scars, scars, large skin pores and crepey skin


Your skin is likely to appear red and feel more sensitive post-treatment. Your skin may feel tight and uncomfortable. Any swelling, itchiness and redness should subside within a few days.

Additionally, because the injections contain your own blood products, there is less risk of an allergic reaction. There is a risk of infection, however its best to use a medically qualified, appropriately trained practitioner.

PRP therapy is unsuitable for anyone who has:

  • Liver pathology
  • A blood-borne disease such as Hepatitis C or HIV
  • Prescribed blood thinning medication
  • Open cuts, abrasions and wounds
  • Active cold sores or skin infections
  • Keloid or hypertrophic scars
  • Poor wound healing

My Patients story  (anon)

Helen carefully applied anaesthetic cream to my face and left it on for about 20 mins

About 10mls of blood was drawn from a vein in my arm. Helen then put the test tube in a machine that spun in order to separate the blood form the plasma. She then carefully injected the good plasma into my cheek just like she would do with filler. The rest of the platelets were carefully syringed over my face acting as a glide to the dermal needling device. So I had the ‘good parts” of my blood injected back in for self rejuvenation with my own  blood components.

It was refreshing to know that I was using my own platelets to rejuvenate my skin so there was nothing artificial. I felt a slight amount of pressure but nothing stingy or painful. The device glided over my face slowly making a slight buzzing noise. It wasn’t painful at all.

The treatment took about 30mins and afterwards a lovely smoothing serum was applied.


What did I feel afterwards!

  • Pink face until the following day
  • Felt a bit like sunburn
  • 2-3 days later my face felt tight and started to flake a bit as my skin was self exfoliating.
  • It was important to keep away form steam rooms, Jacuzzis and swimming to ensure I didn’t get an infection and to stop the gym for 48hours too
  • Avoid any facial treatments for 2 weeks

After about two weeks my face started to glow and my skin tone was more even. I also noticed my large pores had shrunk and I had fewer blackheads. Downtime was about one day and I healed really quickly. After about 4 weeks I noticed my wrinkles were softer and less noticeable.


It was important that I didn’t go for my weekly swim or sauna incase of infection.

I’m looking forward to the next treatment in one month’s time.

What is microneedling ?

So lets talk about microneedling

Microneedling is an aesthetic procedure. Thats why at our clinic in Cheltenham we think it should be carried out by a medical professional.

It involves pricking the skin with tiny sterilized needles causing microscopic injury. The small wounds cause your body to make more collagen and elastin, which heal your skin and help you refreshed exposing new skin cells. It freshens the appearance of skin giving a fresher, strengthened toned appearance. You might also hear it called collagen induction therapy. This controlled injury initiates a skin healing response, resulting in the increased production of collagen and elastin. The end result is thicker, plumper, firmer, smoother skin. Additionally, skincare products are able to penetrate more deeply into the skin, making them more effective.

In Cheltenham we find dermal needling is effective in Collagen induction and treats:

  • wrinkles
  • fine lines
  • scars (including acne scarring)
  • stretch marks
  • large pores
  • hair loss
  • pigmentation

Microneedling can be done in one of two ways:

One option is using a dermaroller. A cylinder with many fine, sharp needles is rolled over the target area. The needles are of various lengths, depending on the qualification level of the practitioner, the area to be treated and type of skin issue.

Alternatively, your practitioner may use a microneedling pen. These are handheld pen-like devices have around 12 fine needles on the tip. Once switched on, these needles move rapidly in and out of the skin, at a depth set by the practitioner.

You can use dermarollers at home, however, medically qualified practitioners can use dermarollers and/or microneedling pens, which penetrate deeper into the skin, giving much better results. Additionally, you are at increased risk of infection due to poor sterilisation and storage, and increased risk of track marks due to needles being blunt.

Microneedling can also be combined with other treatments such as PRP (platelet-rich plasma) therapy, light peels and mesotherapy


  • Microneedling is minimally invasive, requiring no downtime.
  • It’s considered safe for most people who are in overall good health.
  • The procedure isn’t safe for people who use certain acne medications or for women who are pregnant.
  • You’ll experience minor redness and irritation for a few days after the procedure.


  • Total prep and procedure time is about 1 hur 30
  • You’ll need to see a medical professional for safe results
  • You may need at least 3 procedures or more for the best results.


  • Microneedling can cost anywhere from £200-400 per session. The overall costs depend on the size of the area being worked on.


  • It’s considered effective in treating minor scarring related to acne, wounds, and aging. You’ll likely notice brighter, firmer skin, too.
  • Ideal results are achieved after multiple sessions.
  • Microneedling is far more effective than at-home rollers.

The procedure

Following the procedure the skin will look pink for about 24 hours and your face will feel abit like sunburn

No sun exposure and continue to wear an SPF all year round

Are Lip Fillers right for me?

Katy’s Lip filler Journey

Katy had called me a couple of times in my Cheltenham Clinic but due to her fear, she was too scared to make an appointment for a consultation. After about one year of researching, she decided to come and see me.

On meeting Katy, she was quite nervous and she advised me of her concerns. She showed me a picture of her smiling. In this picture she said it looked as if she had no upper lip as it seemed to disappear when she was smiling. She was also concerned about the possible level of pain.

After looking at her teeth I realised Katy had a protruding lower jaw. Her bottom teeth sat in front of her upper teeth) which made her top lip recede.

Katy asked me what I could do to plump up her top lip with filler  and make it visible and not to disappear when she smiled.

1.Will it hurt?

Anaesthetic cream will be applied for 20mins this will anaesthetise the surface of the lips so will minimize the sting . There is also anaesthetic in the product, so the stinging lessens as we proceed

2 . What are the possible side effects?

A Very rare complication is into a vessel so if any severe pain or mottling later. I must be informed immediately

Every medical professional emergency protocol and medication in their emergency bag if there were to be a complication. I advise all my patients to call me text me send picture if they are at all concerned about anything post procedure.

Usually 6-9 months . Given that we are moving our mouths and eating the product may breakdown faster than other fillers. It also depend on your own metabolism

I then proceeded to go through the aftercare

  • No touching for 48hours, No makeup 48 hr No exercise 48 hour
  • No alcohol for 24 hours as it will make you bleed and bruise more
  • Avoid any heat saunas sun steam facial for 2 weeks
  • Use ice on the area for that night onl
  • If lips feel dry use some paw paw with a clean cotton tip
  • Expect swelling the morning after the procedure
  • From 48hours onwards the swelling should subside
  • The product settles in natural HA by 2 weeks

  • Come back in 2 weeks for a review.
  • Avoid kissing or massaging your lips directly after the procedure and possibly for up to 24 hours if it is your first lip filler injection. Do be gentle with your skin while washing for 24 hours.
  • Don’t use and exfoliating agent for 24 hours or any harsh cleaning products
  • Stay hydrated as drinking water will increase the longevity of the treatment

After removing Katy’s makeup, I then used as gentle antiseptic. I utilised my magic anaesthetic cream to help numb the lips. This stays on for 20 minutes to ensure Katy is as comfortable as possible. I proceeded to draw my points and inject Katy’s lips carefully and slowly to reduce the stinging and to be more precise.

The procedure went according to plan with the usual pinpoint bleeding. Katy did have one bruise on her right bottom lip which disappeared within a couple of days. As explained and expected swelling was at its worst the the morning after the procedure. This is due to your circulation not being as efficient whilst you’re asleep and not moving much. During the day as you move around the circulation is more active and swelling subsides.

After 2 weeks we had a review Katy said she was delighted with the results and said she will be back soon. She also expressed that it wasn’t as bad as she thought and the stress ball I gave her to squeeze did take her mind off things. The result was natural see below.
